If your AI template isn’t producing the output you expect - headings are wrong, content repeats, formatting slips, or AI ignores your layout - this guide will help you troubleshoot quickly and refine your templates with confidence.

Common issues and how to fix them

1. AI is generating its own headings instead of using mine

Cause: The prompt is instructing the AI to create headings.

Fix: Remove heading instructions from the prompt.

Use instead: “Fill this section with concise, clinically accurate content. Do not add headings.”

2. AI is ignoring headings that already exist in the template

Cause: The prompt may include conflicting structure instructions.

Fix: Be explicit: “Use the existing headings in the template. Do not create new section titles.”

3. Content in one AI block is repeating in another

Cause: Multiple sections are drawing from the same transcript without boundaries.

Fix: Add a block-specific instruction: “Generate content only for this section. Do not repeat information from other AI blocks.”

4. The AI adds information that wasn’t said

Cause: The prompt is too vague, or AI is inferring details.

Fix: Add: “Use transcript only. Do not infer, assume or add information that was not stated.”

5. Formatting looks inconsistent (bullets vs paragraphs)

Cause: Formatting is not defined clearly in the prompt.

Fix: Specify format: “Use concise bullet points.” or “Write short paragraphs.”
